Tests recorded outside the USFSA standard window will not be accepted and will not be refunded or credited.

 Refunds, credits and transfers are not permitted for any reason. 

 You must include both forms below, labeled according to instruction guidelines,  with your test video link in one email.

Virtual Test Permission forms obtained from your home club should be uploaded in your entryeeze test account.
