Virtual Video Guidelines

For Virtual Testing You will Need to Submit  

1. Name & Likeness Release & Consent form AND

2. Performance Affidavit for Virtual Testing as well as your

3. Home Club permission to test.

4. Your videos filmed WITHIN 14 days of the submission date.
