Virtual Video Guidelines

For Virtual Testing You will Need to Submit  

1. Name & Likeness Release & Consent form

2. Performance Affidavit for Virtual Testing 

3. Home Club Permission to test form (if needed)  HERE 

4. Your video(s) filmed WITHIN 14 days prior to the test date.
